I hereby authorize the American International School-Salzburg and its designated representatives, including administrators, the Health Coordinator, resident-care staff, and other authorized personnel, to obtain medical evaluation, treatment, and care for my child when deemed necessary.

This authorization includes:

• Medical examinations
• Physician consultations
• Diagnostic procedures
• Prescription medications
• Emergency medical treatment
• Hospital treatment
• Emergency transportation by ambulance
• Specialist referrals
• Dental treatment when urgently required
• Other medically necessary treatment recommended by licensed healthcare professionals

I understand that all treatment decisions will be made by appropriately licensed healthcare providers in accordance with Austrian law and accepted professional medical standards.

In the event of a medical emergency, serious illness, accident, injury, or other urgent health concern, I authorize AIS-Salzburg personnel to:

1. Seek immediate medical assistance.
2. Arrange transportation to a medical facility.
3. Authorize emergency treatment when delay could place the student's health or safety at risk.
4. Communicate with healthcare providers regarding the student's condition.
5. Contact parents, guardians, and emergency contacts as soon as reasonably possible. 

I authorize the confidential exchange of medical information regarding my child between:

• AIS-Salzburg administration
• AIS-Salzburg Health Coordinator
• AIS-Salzburg resident-care staff (when necessary)
• AIS-Salzburg teaching staff (when necessary)
• Physicians and healthcare providers
• Hospitals and clinics
• Emergency medical services
• Health insurance providers
• Public health authorities when legally required

Information may include:

• Diagnoses
• Medical histories
• Allergies
• Medications
• Treatment recommendations
• Activity restrictions
• Hospital discharge instructions
• Emergency medical reports
• Other information reasonably necessary for the student's care and welfare

AIS-Salzburg recognizes that medical information is highly sensitive personal data.

Medical information will:

• Be treated as confidential.
• Be shared only with individuals who have a legitimate professional need to know.
• Be maintained in accordance with GDPR requirements and school policy.
• Be used solely for the protection, treatment, safeguarding, and support of the student.

I understand and agree that:

1. Parents or guardians remain financially responsible for medical treatment provided to the student.
2. AIS-Salzburg may initially facilitate payment or administrative arrangements necessary to obtain medical care.
3. Any costs not covered by insurance remain the responsibility of the parent or guardian.
4. It is the responsibility of the parent or guardian to maintain valid health insurance coverage throughout the period of enrollment.

I have read and understood this Medical Treatment Authorization and Exchange of Medical Information Form.

I authorize AIS-Salzburg to obtain medical treatment for my child as described above and permit the confidential exchange of medical information necessary to provide appropriate care and support during the student's enrollment.

This authorization remains valid for the duration of the student's enrollment during the 2026–2027 school year unless revoked in writing.
